Almost nothing, and I would rather say that plainly than dress it up. The build happens the same way it happens for a shop eight miles from me: a call to work out what the site has to do, a first version you can click through inside a couple of weeks, then rounds until it is right.
What it does change is the in person part. I am not going to sit in your break room, and if that is what you want from a designer you should hire someone in Grand Rapids. What you get instead is the person who wrote the code picking up the phone, rather than an account manager relaying it to a team you never meet.
Michigan runs an hour ahead of me, which in practice means your morning email is waiting when I start and I am still at the desk through your afternoon.
